In order to be eligible to receive equipment at no cost through the Connecticut Equipment Distribution Program, please ensure you meet the criteria listed below. Be deaf, hard of hearing, DeafBlind or have difficulty speaking
Be a full-time Connecticut state resident
Be a first-time EDP applicant
Be 5 years old or older (Parental consent is required for those under 18 years old.)

I understand that this program is for the equipment only and that the cost of a wireless plan, telephone plan, or internet plan is not included in the EDP.

I agree to return my device to the Connecticut EDP if I move out of Connecticut within 3 years.

I agree to return my device to the Connecticut EDP for repair if there is an issue with the equipment within 3 years. I understand my device is under warranty.

I agree to notify Hamilton Relay, Inc. within thirty (30) days of any changes in my Connecticut address, phone number or email.
